Patient Portal

Our patient portal is now live!

Through our portal, myhealthrecord.com,  you will be able to view a portion of your child’s medical record, request prescription refills and ask a clinician for NON-URGENT advice.

You can make appointments through our website by clicking here.

NEED TO ENROLL?

To enroll, patients or parents must accept a patient portal invitation sent to their personal email address.The next time you are in the office, the patient care representative will issue you an invitation to join our portal.  If you will not be in the office in the near future and wish to access our portal, please call the office and request that we send you an invitation.

Once receiving the secure link, follow the instructions provided to create a username and password. You must enter the PARENT’s name and demographic information.  This is for security purposes to authenticate who the parent is on behalf of the patient.  If you are 18 or older and are the patient, you will enter your information.  If you have more than one child, you now can view all of your children under your parent account. 

If you do not receive an invitation, please call our office to confirm we have the correct email address on file.  If you are not able to successfully create an account, please call us to verify the information you are entering matches the information on file in your child’s electronic health record.  Example:  If Dad’s name is William in our system, but you enter Will, the system will state “account is not found”, if we have O-Neill but you enter O’Neill, the system will not be able to find you.  Once we verify all information is correct, we will RESEND the invitation.

Important Information
Once you are in the portal, it will automatically default to the last 3 years of data only.  To see all data, click the drop-down menu under “VIEW” and select “All History” and click the blue link to the right to load the data.
