Check Ups for School, Camp, and Sports

 It is that time of the year again to schedule your child’s well exams for
 Pre K, Kindergarten, Summer Camp, and School Sports.

►Keep in mind we only schedule the 4 and 5 year check ups at 8:45 and 1:45, so call today or log on to request an appointment.

►Don’t forget to request forms required for registration such as a 3231 (Immunization Form) and a 3300 (Ear, Eye and Dental Form).

Is your child going to camp this summer or participating in a sport that will require us to complete a physical form?

If so, please call us today to verify that your child is up to date on his/her yearly wellness exam. We request that you bring these forms to your child's well visit, and we will be happy to complete them free of charge. However, any form submitted at other times will require a payment of $5 per form at the time of the request. In addition, as a result of HIPPA regulations, we require a signed release of information in order to complete these forms. We appreciate your patience as we complete your forms. Please allow us 3-5 business days. Once completed, we can have the forms available for you to pick up at the office or we can return the forms to you by mail (with a self addresses stamped envelope) or by fax.
